Alternatively, you can download the Medical History form here online and bring it with you to your appointment. You can make use of our comfortable waiting area, fitted with television and magazines. The dentist will take you through to your treatment room. The comfort and well-being of you is our biggest priority, so don’t be shy to ask questions.

- Two radiograph x-rays and OPG (if required)

WHAT SHOULD I BRING TO MY APPOINTMENT?
- Your health insurance card (if you have private health insurance)
- A list of current medications
Most health funds also have the option of giving you on the spot quotes via HICAPS terminal. Any treatment that you require can be quoted once your treatment plan is completed, showing you how much rebate you will get from your health fund.
